Legacy Empowerment Services is an approved provider through the NJ FamilyCare Housing Supports Program (HSP). Our goal is to empower individuals to secure and maintain safe, stable housing while building confidence, independence, and long-term success. Legacy can meet each individual where they are in their housing journey and provide individualized, person-centered supports based on their needs and goals.

 

Finding and maintaining stable housing can be challenging for many reasons; navigating the rental market, managing daily responsibilities, understanding paperwork, or adjusting to a new environment. Whether someone is transitioning from housing instability or homelessness, or needs support to maintain their current housing situation, these changes can feel overwhelming without the right guidance. Legacy can help you navigate these challenges and reach your housing goals.

 

How We Support You

 

Legacy offers Pre-Tenancy, Tenancy Sustaining, Move-In Supports, and Home Modification & Remediation services. Our Housing Case Managers partner with members to navigate the housing process, build daily living skills, and strengthen their ability to achieve and maintain housing stability.

 

We provide individualized support to help you find and maintain safe, stable housing. 

 

Services include:

 

• Assistance with the housing search and completing applications​

• Completing required documents and communicating with landlords​

• Move-in assistance (when approved) and preparing your new home

• Budgeting, financial planning, and managing paperwork

• Learning daily living skills such as cleaning, organizing, cooking, and home safety

• Developing routines and schedules to maintain your household

• Connecting you with community resources and local supports

• Building independence, problem-solving, and self-advocacy skills

Do I Qualify?

To qualify for the Housing Supports Program, individuals must meet all of the following criteria:

 

Be enrolled with NJ FamilyCare (Medicaid) and receive services through a participating Managed Care Organization (MCO):

 

• UnitedHealthcare

• Horizon NJ Health

• Fidelis

• Aetna

• Wellpoint

Be experiencing one of the following Social Risk Criteria, such as:

 

• Currently homeless

• Living in an unsafe home​

• Staying in a shelter or temporary housing​

• Living in unsafe or unstable living conditions​

• Facing eviction or loss of housing​

• Couch surfing or staying with others temporarily

• Meet the Clinical Risk Criteria, such as:

• Chronic health condition

• Mental health condition

• Substance misuse

• Pregnancy 

• Complex mental health condition from intellectual or developmental disability​

• Victim of partner or domestic violence

• Require assistance with activities of daily living

• Repeated emergency department or hospital use
