Individualized guidance for eligible Members working toward safe, stable, and sustainable housing.

Legacy Empowerment Services provides Housing Supports for eligible NJ FamilyCare Members who are homeless or at risk of homelessness. Support is tailored to the person’s housing needs, level of need, and authorization.

Finding and keeping housing can involve much more than locating an available unit. Applications, eligibility requirements, landlord communication, paperwork, move-in needs, and the responsibilities that come with maintaining a home can all become barriers. Legacy Housing Case Managers help Members navigate those steps with clear, person-centered support.

Our Housing Supports Services

Pre-Tenancy Supports

Support for Members who are preparing to secure housing. Services may include housing search assistance, application support, benefits and eligibility coordination, document gathering, communication related to the housing process, and lease-review support.

Tenancy Sustaining Supports

Ongoing case management after move-in to help Members maintain housing stability. Support includes required monthly phone and face-to-face contacts based on level of need, along with help addressing paperwork, routines, communication, household responsibilities, and other issues that may affect a Member’s ability to remain successfully housed.

Move-In Supports

One-time assistance with eligible furnishings, essential household items, and other approved move-in costs. The purpose is to help a new home become safe, functional, and ready for everyday living.

Environmental Modifications / Home Remediation

Approved modifications or remediation that help make or keep a home safe and accessible. Support is based on the Member’s authorized needs and may address environmental, safety, or accessibility barriers.

Building Stability Beyond Housing

Legacy’s role can continue after a Member finds housing. By helping Members understand responsibilities, stay organized, address concerns early, and build sustainable routines, Housing Supports are designed to create a stronger foundation for long-term stability and independence.
